REAP/Seacoast Mental Health Center

The Referral Education Assistance & Prevention (REAP) program provides community-based, short-term support for older adults who live in New Hampshire, caregivers and family members of older adults, and professionals who work with and for older adults. The team’s goal is to address concerns around mental health, substance use, and cognitive functioning. This program is administered statewide, with services carried out by the ten community mental health centers across NH.
